GENERAL JOB DESCRIPTION:

Under general nursing and medical direction, utilizing standardized procedures, the Nurse Practitioner functions within an organized system for the delivery of health care in association with other members of the health care team and performs specialized nurse practitioner duties in clinical settings. The essential functions include: in collaboration with the physician, manages patient care within the field of specialization; provides health Education and counseling to patients; maintains medical records; participates in nursing and clinic staff meetings and conferences; and may supervise personnel according to clinic structure. The Nurse Practitioner also performs other related duties as assigned/required. These positions are used throughout the Department of Public Health within the specialty areas listed below.

New employees hired in class 2328 Nurse Practitioner for this position(s) will be covered by the SEIU Registered Nurse collective bargaining agreement (CBA) and shall receive a $7,500 bonus payable as follows: $1250 after 6 months of employment, $1250 after twelve months of employment, $1250 after eighteen months of employment, $1250 after twenty-four months of employment, and $2,500 after forty-eight months of employment.

SPECIALTIES: Applicants must indicate the Specialty they wish to apply for on the application. No more than five (5) selections may be made. MINIMUM QUALIFICATIONS: - see "VERIFICATION" section

1. RN LICENSE: Possession of a valid California Registered Nurse (RN) license issued by the California Board of Registered Nursing (BRN); AND

2. NP LICENSE: Possession of a valid California Nurse Practitioner (NP) license issued by the California BRN; AND

3. EDUCATION: Masters Degree in Nursing (MSN) from an accredited college or university; AND

4. CERTIFICATION: Possession of national board certification (or eligibility) as a Nurse Practitioner issued by one of the following recognized national certifying body/organization:

A: ADOLESCENT MEDICINE
Documentation of NP specialization in Family Medicine, Adult Medicine or Pediatric Medicine AND two (2) years (equivalent to 4,000 worked hours) of experience working with adolescents as part of training, volunteering or professional experience.

B: ADULT MEDICINE
Documentation of NP specialization in Family Medicine, Adult Medicine, Acute Medicine or Geriatric Medicine.

C: EMERGENCY DEPARTMENT
Documentation of NP specialization in Family Medicine, Adult Medicine or Acute Medicine AND possession of a furnishing and dispensing number (or ability to pass credential process within 90 days of hire AND possession of suturing/minor-trauma experience or credential (or ability to pass credential process within 90 days of hire).

D: EMPLOYEE HEALTH
Documentation of NP specialization in Family Medicine, Adult Medicine, Occupational Health, Adult or Family Medicine. Applicants must have completed course work in Occupational Health or at least six (6) months (equivalent to 1,000 worked hours) Occupational Health experience.

E: FAMILY PLANNING
Documentation of NP specialization in Family Medicine, Maternity or Midwifery.

F: FAMILY PRACTICE
Documentation of NP specialization in Family Medicine.

G: GERIATRICS
Documentation of NP specialization in Geriatrics.

H: HEALTH AT HOME
Documentation of NP specialization in Family Medicine.

I: JAIL HEALTH SERVICES
Documentation of NP specialization in Family Medicine, Adult Medicine OR Gynecology; AND

Condition of Employment
: Security clearance as issued by the Sheriffs Department is required as a provision to employment for all positions within this specialty area. Eligibles appointed to these positions will be required to obtain the Security clearance directly from the San Francisco Sheriffs Department prior to start work date, which may involve the ability to pass a background investigation. Valid Security clearance must be maintained throughout length of employment. Failure to demonstrate/show proof of possession of valid Security clearance on or before start work date and throughout length of employment may result in dismissal and/or termination of employment.

J: PEDIATRICS
Documentation of NP specialization in Family Medicine or Pediatrics.

K: PSYCHIATRY:
Documentation of NP specialization in Family Medicine or Adult Medicine AND one (1) year (equivalent to 2,000 hours) of experience as a Registered Nurse on a psychiatric inpatient unit, psychiatric emergency service, clinic or mental health center.

L: RAPE TREATMENT CENTER (RTC)
Documentation of NP specialization in Family Medicine or both Adult Medicine and Pediatrics.

M: SEXUALLY TRANSMITTED DISEASE (STD) CONTROL
Documentation of NP specialization in Family Medicine, Adult Medicine, Pediatrics, Womens Health, Obstetrics or Gynecology; AND Six (6) months (equivalent to 1,000 hours) of experience as a Nurse Practitioner, NP Trainee or Physician Assistant providing STD treatment/prevention patient care within the last five (5) years.

N: TRAUMA
Documentation of NP specialization in Family Medicine, Adult Medicine, Acute Medicine, Acute Care or Geriatric Medicine; OR

O: WOMEN'S HEALTH
Documentation of NP specialization in Family Medicine, Adult Medicine, Maternity, Family Practice, Obstetrics or Gynecology.


Condition(s) of Employment (ALL SPECIALTIES)
:

All qualified candidates who have been selected for appointment to positions in all specialty areas must be an eligible billable provider and will be required to meet the following criteria:

Be eligible to participate in Medicare, Medicaid and/or other federal health care programs;

Possess a National Provider Identifier (NPI);

Submit a completed credentialing application and/or required documentation for credentialing; AND

Possess a valid third-party billable provider certification (such as Medicare, Medi-Cal and/or private insurance) OR have submitted a completed billable provider application, along with the required documentation, in order to obtain the appropriate billable provider status.

Failure to meet these criteria and/or Conditions of Employment within two (2) weeks PRIOR to start work date may result in the delay of appointment and/or cancellation of employment offer. Once appointed, all qualified candidates/employees will be required to retain these qualifications throughout length of employment. Failure to demonstrate (show proof) of qualifications shall result in the termination of employment.

Cardiopulmonary Resuscitation (CPR) Certificate Requirement / Provision (ALL SPECIALTIES):

Possession of a valid Cardiopulmonary Resuscitation (CPR) Certificate, issued by the American Heart Association, is required as a provision to employment for all positions. Eligibles who do not possess such a certification may participate in this examination process and be appointed to a position(s), but will be required to obtain the certificate within two (2) weeks of hire date and maintain validity throughout length of employment. A photocopy of the valid Cardiopulmonary Resuscitation (CPR) Certificate must be provided to the appropriate supervisor of the appointment within the allotted time frame. Failure to demonstrate/show proof of possession of valid certification within two (2) weeks of hire date and throughout length of employment may result in dismissal and/or termination of employment.

VERIFICATION:

San Francisco City and County employees do not need to submit verification of qualifying City work experience if experience gained is from the class to which appointed. Credit for experience obtained outside of the employee's class will only be allowed if recorded in accordance with the provisions of Civil Service Rule 110.9.1. City employees relying upon qualifying work experience gained outside the City and County of San Francisco must submit written verification of such experience as described below.


RN AND NP Licensure or Interim Permit
: Verification (proof) of licensure/interim permit must be submitted at the time of filing. Applicants must submit a photocopy of their valid permanent/temporary California Registered Nurse license or interim permit as proof of the required licensure. Failure to meet this requirement may result in rejection of the application.

Certifications: Verification (proof) of National Board Certification should be submitted at the time of filing by submitting a photocopy of their valid certificate/card issued by one (1) of the recognized national certifying bodies/organizations listed in minimum qualification #4. Failure to meet this requirement may result in rejection of the application.

Education: Applicants must submit a photocopy of the appropriate degree and/or official transcript from an accredited college or university evidencing completion of the required field of study and/or course work as proof of qualifying education. Failure to meet this requirement may result in rejection of the application.

Experience: All experience needed to qualify must be submitted on the employer's letterhead and indicate the name of the applicant, job title, dates of employment, hours/week, duties performed and must be signed by the supervisor and/or appropriate representative.

Verification of the required experience may be waived if impossible to obtain. However, the applicant must immediately request a waiver of verification of qualifying experience by submitting a signed statement with the application explaining why verification is unattainable. Please note, requests for waiver of verification of qualifying experience will be considered on a case-by-case basis. SELECTION PROCEDURES:

Depending upon the applicant pool the hiring Department may elect to utilize additional selection criteria on a qualifying or pass/fail basis.

Only those candidates deemed best qualified will continue in the selection process.

TRAINING AND EXPERIENCE EVALUATION
:

The information provided by candidate in the job application, verification, and applicable documents will be evaluated to measure their relative knowledge, skill and ability in job-related areas which may include, but are not limited to: knowledge of the role and scope of practice of the Nurse Practitioner; ability to perform and document physical examinations; ability to gather and analyze data; knowledge of clinical management; knowledge of health care resources; collaborative ability; knowledge of culturally diverse populations; written communication ability and oral communication ability; and duration and relevance of experience (if applicable). Candidates will be placed on the eligible list in rank order according to their final score.

The certification rule for the ranked eligible list resulting from this examination will be Rule of The List. The names of new eligibles will be merged with existing eligibles on the list.
